Rehab-to-Home Program
Our Rehab-to-Home program, under the direction of a dedicated physiatrist, offers a wide array of rehabilitation services for post-acute residents, all with the singular focus of getting your loved one back home to their normal routine. Our expansive gym boasts state-of-the-art equipment, including an ADL suite complete with an integrated kitchen and bathroom, designed to help re-acclimate your loved one to resume daily activities.
Transition To Home
Our ultimate goal is to ensure that you’re ready to transition quickly and safely back to your daily home life. By including, and openly communicating with, your hospital’s primary care physician throughout the decision process and transition period, we ensure your entire care team is working together to proactively monitor your health every step of the way. After discharge, a social worker will check in often to make sure you have what you need and are transitioning well, providing any additional support as needed.
